Consent Agreement Request Form

Complete this form to request a Consent Agreement from Planning Division Staff.

Before completing this form, please ensure that you have reviewed the conditions listed in the Notice of Decision to ensure a Consent Agreement is a condition of consent. Should a condition of consent indicate that the applicant enter into a Consent Agreement with the District Municipality of Muskoka, please contact the District.
